Order Description

research and write a paper that outlines how healthcare technology is impacting disease management (DM) and overall patient outcomes. Giving an historical perspective showing what DM results were prior to the integration of key HIT.

Detail description of the DM technology tools being assessed (purpose, utilization, costs, etc.).
Provide key data that demonstrates disease state prior to and then after utilization of DM technologies (“data” here means the numbers that measure outcomes and overall impact of the DM tool).
Discuss, specifically, how patients interact with DM tools and what other programs practitioners have integrated into their DM programs to improve patient interaction and outcomes.
