Roselinda is a 28 year old Creole speaking woman who arrived in the US  from Haiti 10 months ago.  Her  immigration status is not clear and she is somewhat reluctant to talk about it.   
 She  has  3 children- Gustavo, 8 years old; Rosa, 3 years old and  Joseph, now  3 months old. She is breast feeding the baby and has some questions about whether or not she can maintain this.  She does offer formula as a supplement. Her husband has remained in Haiti and it is not clear if and when he is coming to the US.  Since being in the US, Roselinda has been involved in at least one relationship with a man from her community.  There is some question about the nature of this relationship and Roselinda was the one who ended it. Roselinda has said that she had concerns for her safety and for her children. 
     Roselinda has called the health center requesting an appointment  since neither she nor her children have had any health care since arriving. She did take the baby to the Emergency Room because he had a temperature and was irritable.    She is eager to find a job but because of child care issues she has only worked a couple of days a week for a cleaning service.  
  Her reason for coming today is for a check up for the baby and a required physical for the other children so that they can be enrolled in school . Roselinda herself is in general good health although she appears overweight by observation.
     One important area of concern is the  immunization status of the family.  The older child is in the process of being enrolled in a large urban school although he needs some testing to ensure a correct grade placement.  No one in the family speaks English very well.   Roselinda is also exploring child care options for the younger children.  She does state that they have had immunizations at home but she does not have any documentation.  
Roselinda has a history of a positive PPD at another health center when she arrived.  She thinks she has been positive in the past but is not sure.  Generally she feels well although breast feeding is "making her very tired".
     Gustavo also has a positive PPD and his mother is not clear if this was true in Haiti.  He appears anxious at this visit and keeps talking about school.  
     Roselinda wishes to become a patient at the center and has been told that insurance is not available to everyone. She is asking how to apply.
